| Parental Consent Form & Athletic Pledge | This form must be signed and dated by the parent or guardian granting permission for the student to participate. The reverse side is our "Athletic Pledge" which must be signed by both the parent or guardian and the student. | 10 kb | |
| Athletic Health History Form "yellow form" | Use this form when a physical is due. This form must be completed by the parent or guardian and returned via their son/daughter to the Nurse's Office. |

| Athletic Health History Form "blue form" | Use this form when a physical is NOT due. This form must be completed by the parent or guardian and returned via their son/daughter to the Nurse's Office. |

| Sports Physical Examination Form | This in conjunction with the Health History Form would allow the athlete to go to their own physician to get a physical. The parents would fill out the Health History Form, and the Doctor would complete the Sports Physical Form, and both would have to be returned to the Nurse's Office before the athlete would be allowed to participate. | 55 kb | |
| Extra Help Form | This form is issued to athletes identified by the schools Academic Eligibility Policy as needing extra help. The athlete is to have the form signed by the teacher(s) issuing the extra help for the required number of days, and return to the Athletic Director by Friday of each week. | 48 kb | |
